Can I wear heels or dress shoes with a prosthetic leg?

Short answer

A standard prosthetic foot is aligned to one heel height, so switching to shoes with a different heel can push the knee forwards or back, or tip your posture. A heel-height-adjustable ankle lets you change the ankle angle, often at the press of a button, to match your shoes. It is worth considering if you regularly switch between trainers, dress shoes and sandals.

Why does a prosthesis feel wrong in different shoes?

When a prosthesis is made, the ankle angle and alignment are set for the heel height of the shoes you usually wear. In higher heels, the toe tips downward and the knee is pushed forward; in flatter shoes or bare feet, you feel tipped back. A natural ankle adjusts automatically, but a prosthetic foot with a fixed ankle cannot.

How does a heel-height-adjustable ankle work?

The ankle contains a mechanism that can be unlocked and locked. With the new shoes on, you press a button to release it, put your weight through the foot until the sole sits comfortably on the ground, then lock it again. Many models are designed so users can do this themselves at home. One example is the Pro-Flex LP Align carbon-fibre foot.

The range of heel heights, the weight limit and the amputation levels it suits vary by product. Some are used for above-knee as well as below-knee prostheses, so we check this with you at your consultation.

What does this look like in practice?

One of our below-knee customers chose a heel-height-adjustable carbon-fibre foot with a pin liner. Once it was finished, our customer set the angle for dress shoes and walked in them, then changed into trainers, reset the height for the trainers and walked again. Our customer also climbed stairs naturally without leaning on the handrail.

What should I keep in mind when using one?

  • Set the height when you change shoes — adjust the angle before you start walking.
  • Water — many models are splash-resistant for everyday use. Dry the foot fully if it gets wet, and check the product guidance before using it in the sea or a pool.
  • Weight and activity level — each model has a set weight range and intended activity level.
  • Practice — walk plenty in familiar shoes before moving on to higher heels.

Can I wear very high heels?

The adjustment range differs by product, and very high heels may fall outside it. Tell us which shoes you want to wear at your consultation and we will check which components can work.

Can it be used with an above-knee prosthesis?

As it is a foot component, many models can be used on an above-knee prosthesis. We check how it pairs with your knee at your consultation.

Can I shower or swim while wearing it?

Water resistance varies by product. If it is splash-resistant, avoid pools and seawater; if you are often in the water, consider a separate option such as a waterproof cover.

Is it hard to adjust?

Many models are designed for users to adjust themselves, often with a button. When your prosthesis is fitted, we give you time to practise the adjustment.
